1. I was previously assessed by WNAC and have a request relating to my DSA support
Please contact your funding body (the organisation that you applied to for DSA). You will find this information on your original needs assessment report.

2. I would like to access personal information or documents that you may hold about me
Due to GDPR, we keep records for up to 10 years following a needs assessment. If you last contacted us over 10 years ago, we will no longer hold this information.

3. I want to book a needs assessment and have received a DSA1 letter that confirms I am eligible
