Transition Programme: request specialist support

How to access our support

To access our support, please follow the two-step process below:

Step 1

Please be aware that to implement certain types of support, such as exam arrangements, we require documentary evidence of your condition, which must contain a diagnosis (or working diagnosis).

If you have not already done so, please send documentary evidence of your condition to us via The Student Hub . Please fill the subject of your email as 'Transition Programme 2024 - Evidence for Student Disability and Inclusion Team'. For further information regarding the types of documentary evidence we accept, please view our evidence guidance .

Step 2

Once you have sent us your evidence, please complete the request specialist support form - LINK COMING SOON.

What happens next?

Your form will be reviewed by our team and we will contact you regarding the specialist support available to you. If an appointment is arranged for you, you are welcome to bring your supporter, such as a parent or guardian, with you to join the session.

Do I need to provide evidence of my condition?

If you have documentary evidence confirming your diagnosis (for example, a letter from your doctor/consultant or a diagnostic assessment report), we encourage you to send this to our team via [email protected] so that it can be reviewed by our team.

With documentary evidence, we can discuss certain types of support, such as:

In some cases, detailed evidence from your school or college can be used to put in place temporary reasonable adjustments, so please do send any evidence you may have to our team.
